AB  - A  66-year-old  man  with  a  history  of  coronary  artery  disease,  chronic  renal  insufficiency,  alcoholic  cirrhosis,  hypertension,  and  anxiety  is  brought  to  the  emergency  department  after  a  motor  vehicle  accident.  He  has  traumatic  brain  injury  and  rib  fractures.  After  tracheal  intubation,  he  is  transferred  to  the  intensive  care  unit  (ICU)  for  further  management.  The  patient  appears  agitated  and  uncomfortable.  He  is  tachycardiac,  with  a  heart  rate  of  120  bpm;  hypertensive  with  a  blood  pressure  of  188/72  mm  Hg;  and  tachypneic,  breathing  38  breaths/min.  His  home  medications  include  aspirin  and  clopidogrel.
